NHRA Bahrain: Licensing Guide for Doctors and the BLE 2026

How Bahrain's National Health Regulatory Authority licenses doctors: the physician PQR, the Bahrain Licensure Examination (BLE), attempts, exemptions and PSV.

By MediFlow Editorial TeamLast reviewed: Published: 5 min readOfficial website: NHRA

Every doctor who practises in Bahrain, in a government hospital or a private clinic, needs a licence from the National Health Regulatory Authority (NHRA). The NHRA regulates healthcare provision across both sectors, licenses facilities such as hospitals, medical centres and private clinics, and licenses the healthcare professionals who work in them. For physicians, the central requirement is the Bahrain Licensure Examination (BLE).

The NHRA's physician qualification requirements (PQR) set the standard for licensing. For jobs, visas and daily life, see working as a doctor in Bahrain.

What the NHRA does

The NHRA describes itself as the body that implements and develops health regulations for the government and private sectors in Bahrain. Its supreme authority is the Supreme Council of Health.

For professionals, the Health Profession Regulation Section (HCP) handles registration, licensing and licence renewal for all practising health professionals in Bahrain. It is organised into medical, dental, allied medical, nursing and pharmacy sub-sections, and it assesses the knowledge, clinical skills and professional attributes of professionals entering practice. It also evaluates requests to recognise new medical specialties.

Physician requirements (PQR)

The NHRA physician PQR sets the general conditions for licensing:

  • Education. Academic qualifications are evaluated by the NHRA together with the Ministry of Education's list of recognised universities.
  • Internship. A minimum of one year of supervised clinical practice after graduation, at a university-affiliated or accredited teaching hospital, with at least six months in medical and six months in surgical services. GCC internship certificates approved by the authorised body are accepted.
  • Experience. Experience must have been gained in licensed hospitals and medical centres and is subject to NHRA approval. Requirements depend on the title applied for; new Bahraini graduates are exempt at basic entry level.
  • Good standing. A Certificate of Good Standing, issued by the regulator in your country of practice and not older than six months at the time of application.
  • Primary source verification. Documents such as qualifications, experience, licences and good standing are verified directly with the issuing source.
  • Age. Non-Bahraini physicians must be no more than 65 years old, with certain cases eligible for exemption.

Specialist titles and the NSE

The PQR groups specialist qualifications into tiers. Tier 1 and Tier 2 qualifications are exempt from the NHRA Specialist Examination (NSE), while Tier 3 qualifications obtained outside Bahrain must pass the NSE. The NSE follows the same attempt rules as the BLE.

The Bahrain Licensure Examination (BLE)

The PQR states that all physicians must pass the BLE to obtain a professional licence in Bahrain.

  • Interns may apply for the BLE after completing six months of their internship year, but they cannot be licensed until they provide proof that the internship is complete.
  • Validity. A BLE result is valid for five years.
  • Exemption. Only Bahraini or GCC nationals holding a valid GCC licence are exempt.
  • Attempts. Each physician may attempt the exam four consecutive times within a maximum of three years from the first attempt. Only Bahraini physicians who fail all four attempts can complete at least six months of supervised training in a licensed hospital and then make two more attempts within two years.

How to book

The NHRA's licensure exam page explains the order of steps:

  1. Submit a complete licence application through the NHRA online system. You cannot sit the exam before this.
  2. Once the application is approved, an eligibility code is generated and sent to you through the online system.
  3. Use the code to register with Prometric, following the NHRA's Prometric instructions and video guide.

The NHRA publishes study references for each profession on the same page. It notes that these references are study guides extracted from the Saudi Commission for Health Specialties (SCFHS) website and that exam questions may not come solely from them. MediFlow's app lets you practise Gulf-style licensing questions alongside them.

Gaps in practice

The PQR includes rules for doctors with a discontinuity of practice. Depending on the length of the gap, these can involve passing the NHRA licensing exam and then working under supervision for a period, or completing CPD and clinical training before an exam attempt. Check the gap section of the PQR if you have been out of clinical work.

If your career history has gaps or a non-standard qualification, MediFlow's GCC licensing service can help you prepare at /get-licensed/.

Planning your timeline

Because the BLE can only be booked after your licence application is approved, the sequence matters. A realistic order is: gather documents and start verification, submit the online application, receive your eligibility code, book and sit the BLE, then complete licensing with your employer. Interns can sit the exam after six months of internship, which lets final-year interns get ahead. Once you pass, your result stays valid for five years, so you do not need to have a job lined up on exam day.

Fees (as of October 2026)

The NHRA website lists a Fees page within the HCP section, but the pages we reviewed did not show amounts, so we do not quote any figures. Prometric exam fees are shown during booking. Check the NHRA fees page before applying.

Typical mistakes

  • Trying to book Prometric first. You need the NHRA eligibility code.
  • Using an old good standing certificate. It must be no older than six months.
  • Underestimating the three-year window. Four attempts must fit within three years of the first.
  • Assuming a specialist qualification avoids all exams. Tier 3 qualifications from abroad need the NSE.

Summary
