NMBI Ireland: Registration for Overseas Nurses and Midwives
How the Nursing and Midwifery Board of Ireland registers overseas nurses: application steps, English test scores, decision letter, aptitude test and adaptation.
Every nurse and midwife who works in Ireland must be registered with the Nursing and Midwifery Board of Ireland (NMBI). For nurses trained outside the EU, NMBI is the body that assesses your qualifications, sets the English standard, and decides whether you can register directly or must first complete an aptitude test or adaptation programme.
Ireland has recruited many nurses from India, the Philippines and other countries, and the route is well established. It is still easy to lose months through avoidable mistakes, especially with English tests and documents.

What NMBI does
NMBI protects the public by regulating the nursing and midwifery professions. Its main functions are:
- keeping the Register of Nurses and Midwives, which employers and the public can check;
- setting standards for nursing and midwifery education and approving programmes in Ireland;
- publishing the code of professional conduct and ethics;
- assessing qualifications from abroad;
- handling complaints about registered nurses and midwives.
The register is divided by discipline. You register in the division that matches your training, most often the General Nursing division for overseas applicants.
The route for nurses qualified outside the EU
The overseas process runs in clear stages:
- Create an account and request an application through NMBI's online system.
- Provide English evidence that meets current requirements, unless you qualify through training or practice in a majority English-speaking country.
- Pay the assessment fee and submit documents. Some must come directly from your training institution and your current regulator.
- Qualification assessment. NMBI compares your training with Irish standards for the division you applied for.
- Decision letter. You are told whether you can register or must complete a compensation measure first.
- Aptitude test or adaptation programme, if required.
- Registration. Once all conditions are met, you pay the registration fee and are added to the register.
English language requirements
NMBI accepts three kinds of evidence:
- training in a recognised majority English-speaking country;
- three years of practice in such a country within the last five years;
- an approved English test.
For most overseas applicants, the test route applies. NMBI's current page sets these minimum results:
| Test | Requirement |
|---|---|
| IELTS Academic (or IELTS Academic for UKVI) | 7.0 overall, 7.0 in three components and at least 6.5 in the fourth |
| OET (Nursing) | Grade B in three components and at least C+ in the fourth |
Results must be under two years old and come from a single sitting. NMBI does not combine results from different sittings and does not accept IELTS One Skill Retake, IELTS Online or OET at home. Older NMBI documents show different thresholds, so always go by the current English requirements page.
Compensation measures
If your training differs significantly from Irish standards, NMBI does not refuse you outright; it sets a compensation measure. You usually have a choice between two options listed in your decision letter.
| Measure | What it involves |
|---|---|
| Aptitude test | A written theory exam followed by a practical OSCE that tests clinical skills against Irish standards |
| Adaptation programme | A period of supervised practice in an approved Irish clinical setting, assessed by the host service |
For general nurses, the aptitude test is coordinated by the RCSI Faculty of Nursing and Midwifery. You need a valid NMBI decision letter to book it. The adaptation route depends on finding an approved placement, often through an employer that recruits overseas nurses.
Choosing between them
- The aptitude test suits nurses who prefer to prepare independently and want control over timing.
- The adaptation programme suits nurses who already have a recruiting employer that offers placements, since you gain Irish clinical experience at the same time.
Whichever you choose, keep your decision letter in date. If it expires before you finish, you may have to restart parts of the process.
Midwives and other divisions
The process for midwives and for nurses in other divisions follows the same stages, but the assessment compares your training with Irish standards for that specific division. A nurse trained in general nursing who also practised in mental health, for example, is assessed for the division that matches the formal qualification, not for work experience alone. If your training combined several fields, NMBI may register you in one division and ask for further evidence or a compensation measure for another.
After registration
Registration is renewed every year by paying the annual retention fee. NMBI expects registered nurses and midwives to keep their practice up to date and to follow its code of professional conduct and ethics. If you move between employers, your registration stays with you, but your employer will check it each time. If you leave Ireland, keep your registration active or follow NMBI's process to come off the register voluntarily, because a lapse can make later return more complicated.
Fees (as of October 2026)
NMBI's overseas application form lists a โฌ350 non-refundable assessment fee for first-time applicants. A separate registration fee applies once you are approved, and an annual retention fee keeps you on the register. The aptitude test or adaptation programme has its own cost, set by the provider. Some employers cover part of these costs; read your contract carefully.
Fees change, so check NMBI's registration fees page before paying.
Checklist before you apply
- English result in date, from one sitting, meeting the current thresholds.
- Transcripts and syllabus with clinical and theory hours, ideally sent directly by your school.
- Certificate of good standing from every regulator you have been registered with.
- Identity documents and any name-change evidence.
- Budget for the assessment fee, registration and a possible aptitude test.
- Employer contact, if you plan on the adaptation route.
Typical mistakes
- Booking IELTS Online or OET at home. These are not accepted.
- Combining two test sittings. NMBI needs one sitting.
- Letting the test expire while waiting for documents.
- Sending documents yourself when NMBI requires them direct from the institution.
- Ignoring the decision letter deadline.
Preparing for the aptitude test
The theory part tests Irish nursing practice, safe medication management, professional conduct and clinical reasoning. The OSCE checks practical skills and communication. The MediFlow Exam Prep app and the NMBI practice track help you revise theory with exam-style questions before you move to skills practice.
