Socialstyrelsen: Swedish Doctor Licence (Legitimation) 2026

How Socialstyrelsen licenses doctors in Sweden: EU/EEA route, the five steps for non-EU graduates, the knowledge test, practical service and special appointments.

By MediFlow Editorial TeamLast reviewed: Published: 5 min readOfficial website: Socialstyrelsen

In Sweden the title läkare is protected and the right to practise is tied to a licence, called legitimation, issued by the National Board of Health and Welfare (Socialstyrelsen). Socialstyrelsen decides which route you follow based on where you studied, runs the licence applications, recognises specialist qualifications and keeps the national register of licensed health professionals that employers check.

What Socialstyrelsen does

For doctors, Socialstyrelsen:

  • assesses foreign medical education and decides what you still need to complete;
  • grants the licence and recognises specialist qualifications;
  • issues special appointments that allow time-limited work in a named post;
  • maintains the register of licensed staff.

Applications are made through its online licence service, with paper forms available for some steps.

Doctors educated in the EU/EEA

If your medical degree comes from an EU/EEA country, Socialstyrelsen handles your application under the European recognition rules. It also offers EU/EEA graduates the option of applying for the Swedish general internship (AT). Separate rules apply to UK diplomas: those issued before January 2021 may still qualify on the same basis as before Brexit.

You still need to show competence in Swedish, Danish or Norwegian. In practice employers will expect fluent Swedish for patient work.

The route for doctors educated outside the EU/EEA

Socialstyrelsen describes a stepwise route. You do not have to finish one step completely before preparing for the next, but the order below is how most doctors move through it.

Step 1: Assessment of your education

You apply to have your medical degree assessed (granskning). If the degree can form the basis for a Swedish licence, you receive a decision that lists the remaining steps and allows you to sit the knowledge test.

Step 2: Swedish language

You must prove Swedish skills with documentation that Socialstyrelsen accepts. It assesses language skills in Swedish, Danish or Norwegian. Because the knowledge test and the practical service are in Swedish, language is the real bottleneck for most applicants.

Step 3: The knowledge test (kunskapsprov)

The test checks that your medical knowledge and clinical skills match a Swedish graduate's. It has two parts:

  • a theoretical part, covering broad medical knowledge;
  • a practical part, assessing clinical skills with patients and scenarios.

Umeå University runs the test nationally on behalf of Socialstyrelsen, and it is held in Swedish. Several universities, including Lund and Gothenburg, run complementary education programmes for doctors with a degree from outside the EU/EEA; some use results from the theoretical part in their admissions.

Step 4: Practical service

After the test you complete supervised practical service in Swedish healthcare. You need a special appointment to practise during this service, and your supervisor must be a trained supervisor with a Swedish licence. At the end, the supervisor and the head of the unit sign a certificate that you send to Socialstyrelsen.

Step 5: Course in Swedish health regulations

The final requirement is a course on the laws and rules that govern Swedish healthcare: patient safety, records, confidentiality and the duties that come with a licence.

When all steps are complete, you apply for the licence.

Special appointments explained

A special appointment (särskilt förordnande) is a temporary permission to work as a doctor in a specific post for a limited period. It does not let you use the protected title läkare except while you are in prescribed practical service. Points to know:

  • A special appointment cannot be given to someone who already meets the criteria for a licence; that person must apply for the licence.
  • Either the employer or the doctor can apply. If the doctor applies, an employer certificate must confirm the post and period.
  • Private healthcare providers cannot decide on special appointments themselves; they must apply to Socialstyrelsen.
  • From 1 February 2026, medical students need at least ten completed semesters to qualify for a special appointment.

Fees (as of October 2026)

Socialstyrelsen charges fees for licence applications and certificates through its online service. The amounts depend on the application type; check the current figure in the service before you pay. Language courses, the knowledge test and complementary programmes have their own costs, and universities charge tuition to citizens of countries outside the EU/EEA and Switzerland.

Checklist before you apply

  • Degree documents ready for the assessment, with certified translations where needed.
  • Swedish plan that reaches professional level, since the test is in Swedish.
  • Test preparation for both parts, not only the theoretical one.
  • Supervisor and post lined up for practical service, so the special appointment can be issued.
  • Regulations course scheduled before your licence application.

Typical mistakes

  • Revising only theory. The practical part tests clinical skills and communication.
  • Starting practical service without a special appointment. You need it before you treat patients.
  • Using the title läkare too early. It is protected.
  • Ignoring complementary programmes. They can be a structured alternative for some applicants.
  • Paying in a way that fails. Socialstyrelsen has reported payment disruptions with Swish in autumn 2026 and recommends paying by card.

Preparing for the knowledge test

The theoretical part rewards broad, steady revision across all core specialties. You can practise clinical questions in the MediFlow Exam Prep app while you work on your Swedish, then switch to Swedish-language material closer to the test.

A realistic planning sequence

Most doctors who succeed treat the route as one long project rather than five separate hurdles. A workable sequence looks like this:

  1. Months before applying: start Swedish lessons and gather certified copies of your diploma, transcripts and any internship certificates.
  2. Application: submit the assessment application and use the waiting time for language study, not only medical revision.
  3. Language milestone: reach a level where you can read clinical texts and talk to patients comfortably, because both test parts happen in Swedish.
  4. Knowledge test: prepare the theoretical part with broad question practice, then train history-taking, examination and explanation for the practical part.
  5. Practical service: contact regions and hospitals early. Employers need time to arrange a supervisor and apply for your special appointment.
  6. Regulations course and licence application: finish the course, collect the supervisor's certificate and apply.

After the licence

A Swedish licence lets you work independently as a doctor, but specialist status is separate. If you already hold a specialist qualification from abroad, you can apply to Socialstyrelsen for recognition; otherwise you enter Swedish specialist training through an employer. Your licence also comes with duties: Socialstyrelsen and the health inspectorate can act if a doctor's practice puts patients at risk.

Next steps

  1. Read Socialstyrelsen's licence pages and choose läkare in the online service.
  2. Apply for assessment of your education.
  3. Compare the other Nordic routes: Norway, Denmark, Finland.
  4. Read working as a doctor in Sweden.

Assessment of your degree, proof of Swedish, a knowledge test with a theoretical and a practical part, supervised practical service and a course in Swedish health regulations.

Who runs the knowledge test?

Umeå University runs the test nationally on behalf of Socialstyrelsen. It is held in Swedish.
