Medical Licence in Florida for IMGs: Board Rules and Steps

How IMGs get a Florida physician licence: Florida Board of Medicine pathways, two years of residency, exams, fingerprints, fees (October 2026) and steps.

By MediFlow Editorial TeamLast reviewed: Published: 5 min readOfficial website: Florida Board of Medicine

Florida has a fast-growing population, a large older patient base and steady demand for physicians. Its licensing rules for international medical graduates are relatively straightforward compared with some states: the key requirement is an approved residency of at least two years in one specialty area. This guide explains the route as the Florida Board of Medicine describes it. Rules and fees change, so confirm current requirements on the Board's website before applying.

Who regulates physicians in Florida

The Florida Board of Medicine sets standards for allopathic physicians and decides on licence applications, supported administratively by the Florida Department of Health. Osteopathic physicians apply to a separate osteopathic board.

Licensure pathways for international graduates

The Board describes two main pathways for international graduates applying for licensure by examination.

PathwayWho it is forRequirements
International allopathic graduateGraduates of an allopathic medical school outside the US and CanadaValid ECFMG certificate and an approved residency of at least two years in one specialty area
"Fifth pathway"Graduates who completed all formal requirements of a foreign school except the internship or social service componentPassing the required parts of the national exam, an academic year of supervised clinical training, and an approved residency of at least two years in one specialty area

The fifth pathway is rare today; nearly all IMGs follow the first route.

What "two years in one specialty area" means

The two years should be in the same specialty, normally in an ACGME-accredited program. Two unrelated preliminary years in different specialties may not meet the requirement. If you changed programs or specialties, ask the Board how your training will be counted before applying.

Examination requirements

The standard route is passing all parts of a national licensing exam, which for most IMGs means USMLE Steps 1, 2 CK and 3. The Board also describes alternative routes for experienced physicians already licensed elsewhere in the US or Canada for many years, which involve additional examinations such as the Special Purpose Examination (SPEX). These routes are for established practitioners, not new graduates.

For the exams themselves, see the USMLE Step 1 guide and the Step 2 CK guide.

The pathway step by step

  1. Medical degree from an international school.
  2. USMLE Step 1 and Step 2 CK.
  3. ECFMG Certification.
  4. Residency in an accredited program; see the IMG pathway to US residency.
  5. USMLE Step 3.
  6. Complete at least two years of approved residency in one specialty area.
  7. Apply to the Florida Board of Medicine for licensure by examination.

Application checklist

  1. Apply online through the Department of Health licensing portal and pay the application fee.
  2. Use FCVS if possible. The Board notes that primary-source verification of credentials, including your ECFMG certificate, is available through the FSMB's Federation Credentials Verification Service, and that not completing FCVS before applying may delay licensure. See FSMB and US state medical boards.
  3. Translations: documents in another language need a literal translation from an acceptable translator.
  4. Fingerprinting: complete electronic fingerprinting through an approved Livescan provider using the Board's correct identifier. If the wrong identifier is used, the Board will not receive the results. You pay the screening costs, and no licence is issued without results.
  5. Training verification: each program verifies your postgraduate training.
  6. Exam scores: request your USMLE transcript.
  7. Pay the initial licence fee when the Board approves your application. Residents and fellows can pay the reduced fee with a letter from their training director and are exempt from the NICA fee.
  8. Respond quickly to any deficiency notices.

Fees (status October 2026)

Initial licence fee for residents and fellows (with training director's letter)$205

The Interstate Medical Licensure Compact

The Compact offers expedited licensure in member states for physicians who already hold a full, unrestricted licence in a member state of principal licence, completed accredited graduate medical education, and passed each component of the USMLE or COMLEX within a limited number of attempts. Check the Compact's current list of member states and eligibility rules.

Visa considerations

State licensing and immigration are separate. Most IMGs train on J-1 or H-1B visas, and a full Florida licence after two years of residency does not change visa obligations.

Common mistakes

  • Two years split across unrelated specialties, which may not count as two years in one specialty area.
  • Using the wrong fingerprint identifier, so the Board never receives the results.
  • Skipping FCVS, which can delay verification.
  • Missing translations or translations that are not literal.
  • Paying the full initial licence fee as a resident when the reduced fee applies.
  • Not disclosing past issues on the application.
