Medical Licence in Texas for IMGs: TMB Requirements and Steps
How IMGs get a Texas physician licence: Texas Medical Board education pathways, two years of training, jurisprudence exam, provisional route and application steps.
Texas has a large and growing physician workforce, many residency programs and a significant number of international medical graduates in practice. Its licensing rules for IMGs have two distinctive features: an education equivalence requirement on top of ECFMG Certification, and a state jurisprudence exam that every applicant takes. This guide explains the route as the Texas Medical Board describes it. Rules change, so confirm the current requirements on the Board's website before applying.

Who regulates physicians in Texas
The Texas Medical Board licenses and disciplines physicians (both MDs and DOs) and certain other professions. It reviews applications, sets examination and training standards, and runs the jurisprudence exam requirement.
The pathway for IMGs
- Medical degree from an international school.
- USMLE Step 1 and Step 2 CK. See the USMLE Step 1 guide and the Step 2 CK guide.
- ECFMG Certification to enter accredited US training.
- Residency in an accredited US program; see the IMG pathway to US residency.
- Postgraduate permit if you train in Texas. Residents in Texas programs need a training permit from the TMB for their training.
- USMLE Step 3.
- Meet the IMG education requirement (see below).
- Pass the Texas Medical Jurisprudence Examination.
- Apply for the full licence after completing at least two years of qualifying training.
The IMG education requirement
In addition to the requirements for US and Canadian graduates, the TMB requires international graduates to show one of the following:
| Option | What it means in practice |
|---|---|
| ABMS or BOS specialty board certification | Certification by a US specialty board recognised by the American Board of Medical Specialties or the osteopathic Bureau of Osteopathic Specialists |
| Substantial Equivalence list | Your medical school appears on the TMB's list of schools found substantially equivalent to US schools |
| FCSA evaluation | The Foreign Credentials Service of America has evaluated your medical education and found it equivalent to US medical education |
Many recent residency graduates are not yet board certified when they first apply, so the school list or the FCSA evaluation is often the practical route. Start the evaluation early because it takes time to gather documents from your school.
Postgraduate training
International graduates must complete one of the following:
- two years of continuous, progressive accredited training (normally the first two years of an ACGME-accredited residency),
- Board-approved training,
- teaching under a Faculty Temporary Licence in Texas, or
- a combination of these.
As an alternative, the Board may accept five years of practice under an unrestricted full licence in the US with no disciplinary action in any state.
"Progressive" means each year builds on the previous one (for example PGY-1 then PGY-2), rather than repeating the same level. Observerships, research and unaccredited clinical work generally do not count.
The Texas Medical Jurisprudence Examination
All applicants must pass the jurisprudence exam, which covers Texas laws and TMB rules relevant to medical practice, such as prescribing rules, reporting duties, patient records and professional conduct. It is open book. Read the Board's study materials before booking; candidates who skim them often underestimate the detail.
Exam attempt and time rules
Texas has rules on how many attempts at the USMLE steps it accepts and how long you may take to complete the exam sequence, with some exceptions (for example for physicians who are board certified). Check the Board's current examination requirements before you take or retake any step; failing to meet them can block licensure even after residency.
The provisional licence pathway
Texas has a provisional route for certain physicians who completed their training outside the US. Under it, physicians practise under supervision with an Initial Provisional Licence and then a Second Provisional Licence, within a set overall time frame, before they can apply for a full licence. Eligibility criteria are specific (training, experience, employment and supervision conditions), so this is not a general alternative to residency. Read the Board's provisional licence page carefully if you think you may qualify.
Step-by-step application checklist
- Create an account in the TMB online application system.
- Set up an FCVS profile with the Federation of State Medical Boards to collect primary-source verified credentials. The FCVS profile can be reused for other states; see FSMB and US state medical boards.
- Request ECFMG verification.
- Request your USMLE transcript.
- Arrange the IMG education requirement (Substantial Equivalence list confirmation or FCSA evaluation, or proof of board certification).
- Have each training program verify your postgraduate training.
- Pass the jurisprudence exam.
- Complete fingerprinting and background checks.
- Pay fees. Fees change; check the Board's current fee schedule.
- Respond promptly to deficiency notices.
Timeline example for an IMG
| Stage | What happens |
|---|---|
| Before residency | USMLE Step 1 and 2 CK, ECFMG Certification, Match |
| Start of residency in Texas | TMB postgraduate permit |
| PGY-1 to PGY-2 | Pass Step 3; start FCSA evaluation or check Substantial Equivalence list |
| During PGY-2 | Pass jurisprudence exam, prepare full licence application |
| After two years of progressive training | Eligible for a full Texas licence if all other requirements are met |
Visa considerations
Visa status is federal and separate from state licensing. Most IMGs train on J-1 or H-1B visas. A full licence after two years of training does not change visa obligations; J-1 physicians usually still need to finish training and fulfil or waive the home-residence requirement.
Common mistakes
- Not starting the IMG education requirement early. Collecting school documents and an evaluation can take months.
- Assuming one year of training is enough. That rule applies to US and Canadian graduates, not to IMGs.
- Ignoring exam attempt and time rules until after residency.
- Underestimating the jurisprudence exam.
- Name mismatches across passport, diploma and exam records without supporting documents.
- Not disclosing past issues.
Preparing for the exams
Strong USMLE results open residency doors; Step 2 CK scores carry particular weight now that Step 1 is pass/fail. MediFlow Exam Prep offers USMLE-style questions with explanations and free sample questions on the course page linked here. Our high-yield pharmacology and high-yield emergency medicine guides cover heavily tested topics.
Other states
Compare Texas with California, New York and Florida. If you plan to work in several states, check whether each state takes part in the Interstate Medical Licensure Compact.

Who issues medical licences in Texas?
The Texas Medical Board (TMB) licenses both MD and DO physicians in Texas.
How much residency training do IMGs need in Texas?
The TMB requires international graduates to complete two years of continuous, progressive accredited training, or Board-approved training, or a combination with teaching under a Faculty Temporary Licence. Five years of practice under a full unrestricted US licence without discipline may be accepted instead.
What extra education requirement applies to IMGs?
IMGs must show one of the following: ABMS or BOS specialty board certification, graduation from a school on the TMB's Substantial Equivalence list, or a credential evaluation from the Foreign Credentials Service of America finding the education equivalent to US medical education.
What is the Texas Medical Jurisprudence Examination?
An open-book exam on Texas laws and Board rules that govern medical practice. All physician licence applicants must pass it in addition to the national licensing exams.
Is there a route for physicians who trained abroad without US residency?
Texas has a provisional licensing pathway for certain physicians trained outside the US, under which they practise under supervision with an initial and a second provisional licence before applying for a full licence. Eligibility is narrow, so check the TMB website.
